I have messed with it very thoroughly,
but my /etc/security/access.conf file has

-:ALL EXCEPT root:ALL

I think I may have also tried it with a username after root,
but don't recall now.  I didn't need to restart sshd after
I made the changes to the files (/etc/security/access.conf,
/etc/pam.d/ssh and /etc/pam.d/login.

FWIW  I found the example on this page
