I use IET (http://iscsitarget.sourceforge.net/) compiled for CentoS 5.2.

It is really simple to use and configure.

I have setup storage using openfiler as an iscsi-target. I use gigabit
interfaces, two links in channel bonding, and the iscsi storage serves
both a physical and a virtual cluster of XEN VMs.
