Maybe this will help, its a vgscan run with debugging enabled. I've attached
the output b/c its rather long.

This looks a little suspect:

<4444> pv_read_uuidlist -- CALLED with /dev/hdh3
<55555> lvm_check_uuid -- CALLED with uuidstr:
"ws4LAqXeqzGhRJZQWXaCNl4J6JRB3Cro"
<55555> lvm_check_uuid -- LEAVING with ret: 0
<55555> lvm_check_uuid -- CALLED with uuidstr:
"WMWlDyT2fH5C588PEumOzyiItO9O7Y0t"
<55555> lvm_check_uuid -- LEAVING with ret: 0
<55555> lvm_check_uuid -- CALLED with uuidstr:
"1CovqiEp1ZVcazc4BsqdKAYKU2cCo8UD"
<55555> lvm_check_uuid -- LEAVING with ret: 0
<55555> lvm_check_uuid -- CALLED with uuidstr:
"WVy987Bcf3J50Nbr0eO4aRk5pmpfjqkR"
<55555> lvm_check_uuid -- LEAVING with ret: 0
<55555> lvm_check_uuid -- CALLED with uuidstr: ""
<55555> lvm_check_uuid -- LEAVING with ret: -1

And it repeats the last 3 lines MANY times.
