Request you to help me with the issue resolution.
 I am getting vgchange addtag error sometimes when my vg is trying to come online. Not able to get the reason behind this. It's a random behaviour and may not occur next time when the same package fails over to this node and will successfully come online.

vg_tag
Attempting to addtag to vg TestVG21...
vgchange --addtag error:
WARNING: Locking disabled. Be careful! This could corrupt your metadata. /dev/mpath/mpath89: write failed after 0 of 4096 at 8192: Operation not permitted
Activating volume group TestVG21 .
sg_filesystems
sg_check_and_mount
Checking filesystems:
/dev/TestVG21/TestLV21
e2fsck 1.39 (29-May-2006)
e2fsck: No such file or directory while trying to open /dev/TestVG21/TestLV21

The superblock could not be read or does not describe a correct ext2
filesystem.  If the device is valid and it really contains an ext2
filesystem (and not swap or ufs or something else), then the superblock
is corrupt, and you might try running e2fsck with an alternate superblock:
e2fsck -b 8193 <device>

In the similar environment sometimes i also get this error:

mount: unknown filesystem type 'ext3'

Hi,

I'm more on the management side so probably can't help you much with
technical issues.  Have you emailed your question to one of the upstream
mailing lists?  I'm sure Fabio or someone upstream can help you with the
