[Pki-users] Re-instating Revoked Certificate

For the revoked cert with reson "Certificate is on hold" you should see 
an "Off Hold" button right below "Details" when you list (or search) the 
certificate.  Click on the "Off Hold" button then your cert will be 
taken off hold.

I hope we have it in the doc somewhere.
