when you see not being redirected from / to /dashboard, puppet-modules 
didn't add the redirection.

Taking this and your internal server error, I have the stronger feeling, 
your dashboard was installed, but puppet modules didn't do the after 
install configs, which were necessary for horizon to work, when you got 
a package produced between last Friday and yesterday morning.
That issue was patched and should be fixed in a later version.

I can not say anything, why puppet modules failed.
