increasing memory usage

Has anyone experienced on their machines the amount of memory and swap
used to be ever increasing?  This is more of a server problem because it
takes a week for all the memory and swap to run out (and then, of
course, the system hangs).

On this note, are there any tools for examining the contents of swap in
order to see which processes are holding it?
