rpm uses a Berkeley DB database to keep track of installed packages,
located in /var/lib/rpm.

Try installing the db4-utils package (4.0.14 is latest), and doing
	db_verify /var/lib/rpm/Packages

If db_verify reports errors, then the next step is to do (as root)
	cd /var/lib/rpm
	mv Packages Packages-ORIG
	db_dump Packages-ORIG | db_load Packages

Once the Packages database is OK, the associated indices are regenerated
by doing
	rpm --rebuilddb
