I couldn't add any video device model, after I add hardware and choose a video device model, apply button is disabled! but for adding storage or Graphics for example it works properly and for adding usb also not working.

This is likely a xen or qemu bug. I'd recommend filing a bug against F18 xen,
and provide the XML from sudo virsh dumpxml $vmname. You can try changing the
video device model of the VM using virt-manager, and restart the VM, it could
be a busted device model.
