Abstract: Urgent treatment decisions are made at unpredictable times, making immediate access to advance directive and advance care plan information an essential component to providing personalized care.  Erosion of trust in the healthcare system occurs when treatment is not informed by an individual’s personal values, goals, and preferences.  Most electronic record systems were unable to locate and retrieve previously created digital and paper documents from outside the treating provider’s setting.  The unique healthcare conditions brought on by the pandemic underscored the critical need for accessible, verifiable digital advance healthcare decision documents.  During the height of the crisis, many individuals found themselves during a transition of care without family and/or friends at their side to inform and influence medical care decisions. Thus, COVID not only highlighted the importance of advance care planning for healthy adults, patients, and their families.  It also heightened medical professionals’ expectations for seamless data access to this critical information.

These modern design principles must be employed to create comprehensive healthcare solutions, accommodate infrastructure growth and rapidly increasing number of devices, enable "healthcare follows the patient" movement, and create "connected hospitals." All this has increased multi-tenancy and a need to process sensitive data at remote and point of care locations, while in a more challenging cybersecurity environment.
